1. HEALTH & SAFETY DISCLOSURE
• Consumed alcohol or recreational drugs within the last 24 hours?  Yes / No
• Taking blood-thinning medication or medication that may affect healing? Yes / No
• Condition affecting blood clotting, healing, or immune function? Yes / No
• Diabetes or other condition that may affect healing? Yes / No
• Known allergies/sensitivities to ink, pigments, latex, adhesives, or skincare products? Yes / No
• Currently sick, feverish, infected, or otherwise unwell? Yes / No
• Pregnant or breastfeeding? Yes / No
• History of abnormal scarring or keloids? Yes / No
 

2. INFORMED CONSENT
I voluntarily request and consent to receive the tattoo described above. I understand that tattooing involves inserting pigment into the skin with needles and may cause pain, discomfort, bleeding, redness, swelling, scabbing, peeling, or other temporary effects. I understand that tattoos are permanent and removal may be difficult, expensive, time-consuming, and may not completely remove the tattoo. I have had the opportunity to ask questions and understand the answers provided. I understand that healed results vary based on skin type, placement, aftercare, sun  exposure, healing, and other individual factors, and that a specific healed result cannot be guaranteed.

3. RISKS & POSSIBLE COMPLICATIONS
I understand that risks may include pain or discomfort; bleeding; redness or swelling; infection; allergic or inflammatory reactions; changes in pigmentation; scarring or keloid formation; poor or uneven healing; ink migration or fading; and changes in appearance over time. I agree to disclose relevant health conditions, medications, and allergies before the procedure.

4. HYGIENE, SAFETY & AFTERCARE
I understand that appropriate hygiene and infection-control procedures will be used, including sterile, single-use needles and disposable materials as appropriate. I agree to follow the artist's instructions during and after the procedure. I understand that proper aftercare is important for healing and that failure to follow aftercare instructions may affect the tattoo and increase the risk of complications. If I experience concerning signs of infection or a serious reaction, I understand that I should seek appropriate medical attention.

5. DESIGN, PLACEMENT & ARTISTIC JUDGMENT
I confirm that the design, placement, approximate size, and overall concept have been discussed with the artist. I understand that tattooing is an artistic service and the final tattoo may differ slightly from a reference image, drawing, digital rendering, or verbal description. I approve the final design and placement before the tattoo is applied.

6. PHOTOGRAPHY & PORTFOLIO CONSENT

I understand that the artist may photograph or record my tattoo. If I consent, I authorize use of those images/videos for the artist's portfolio, website, social media, advertising, and promotional purposes. I may request that identifying information not be included with published images.

7. CLIENT RESPONSIBILITY & RELEASE
I confirm that the information I have provided is truthful and accurate. I am voluntarily receiving this tattoo and am not under the influence of alcohol or drugs that would impair my ability to provide informed consent. I have disclosed relevant health conditions, medications, and allergies. I understand the permanent nature of tattooing and have had the opportunity to ask questions. I understand the deposit price and estimated price. I understand that if a touch up is needed within the first 60 days after the date this consent form is signed and tattoo is completed, an additional $150 is required to touch up, and anytime passed will need to rebook as a new tattoo appointment. I understand not completing a tattoo will require another deposit fee and/or not limited to another paid session. I understand, if I reschedule an appointment, I am limited to one free reschedule when artist is available. Anything after I understand I will rebook another deposit.

Release of Pure Tattoo LLC and Associated Businesses/Partners: To the extent permitted by applicable law, I acknowledge the ordinary risks associated with receiving a tattoo and agree that I will not hold Pure Tattoo LLC, Rexii, Tip Off Barbershop, or any other disclosed business partner, host business, affiliated business, owner, operator, employee, contractor, or representative responsible for complications resulting from information I failed to disclose, failure to follow aftercare instructions, or circumstances outside their reasonable control. This release does not waive rights or responsibilities that cannot legally be waived under applicable law.

Important: This release is intended to address ordinary risks and disclosed conditions; it does not excuse negligent or unlawful conduct or override rights that cannot
legally be waived.
